There is an option marked Field Name Header on the Export dialog that pops up. Turn it on.

Okay, I've gotten that far.  Where i'm getting stuck is in MS Word.  I can select the label settings, but I can't use my saved file from Sesame.  When I select it, the address labels read "next record".

Which Data source should I select?

In word select open data source,in the open data source box that comes up, change file type to all files, then find the file you exported in Sesame, Then the confirm data source box comes up choose ms-dos text,click o.k., the setup main document box comes up click, Choose the labels to use,Then a box comes up to let you insert fields, choose the fields to add,then mail-merge helper comes up choose merge, merge box comes up choose merge, and your labels should appear.

I am using MS Word 2003

Thanks for trying to help me but i've spent all morning trying to figure this out.  I guess I have to actually call in....which I didn't want to do...

After you select your Export file you will get a box that says Mail Merge Recipients.
Click Ok.
Your labels will all say "Next Record.
Click "Next: Arrange your labels" to go to Step 4.
Click in the first label.
Click More Items, it is on the left hand side.
Insert all your fields.
Click Close. 
Arrange your fields so they are on the lines the are supposed to be on. 
Click the Update all labels button.
Click Next: Preview your labels.
Click Next: Complete the merge.
You can then print the labels or edit them.
