I have a Profile DB(KeyNumber,Last Name, First Name, DOB, Weight,Gender)

I have a FA Db.  In the FA data entry form, I need to populate the fields ((KeyNumber,Last Name, First Name, DOB, Weight,Gender) from the Profile DB.

I want it user friendly but simple.  Something like a popup where the user selects by the First & Last Name then it populates the rest of the fields.  I'll take any suggestions.

I have a database where you enter the project and apt (001-16-1) and it gets the info from another database and populates several of the fields all done with @lookups.
